What is the Difference Between Enucleation and Marsupialization

The key difference between enucleation and marsupialization is that enucleation is carried out in the eye to remove the entire eyeball, while marsupialization is carried out in the labia to treat Bartholin cysts.

Enucleation and marsupialization are surgical procedures. Enucleation involves the removal of the entire eyeball and its intraocular contents. This involves the separation of all tissue connections with the eyeball or the globe, the eye muscles, and lids or orbit. Marsupialization helps to treat Bartholin cysts. Bartholin cysts are fluid-filled lumps formed on two glands on either side of the labia. These glands secrete fluids to lubricate the vagina.

What is Enucleation?

Enucleation is a surgical procedure to remove the entire eyeball, leaving the lining of the eyelids and muscles of the eye. Indications for enucleation are high suspension for intraocular malignancy, trauma without visual potential, painful eyes, blindness, microphthalmos, severe infection, and sympathetic ophthalmia. Enucleation is performed as an outpatient procedure under general anesthesia. A retrobulbar block of the local anesthetic, along with epinephrine, is given for hemostasis and postoperative pain management.

Enucleation is performed as a final action following diseases or severe injuries to an eye. Diseases that lead to this procedure are end-stage glaucoma or ocular tumors such as uveal melanoma or retinoblastoma. Enucleation is also performed when an eye is severely injured beyond repair and is blind and painful. During such incidents, the procedure improves the quality of life by relieving pain, restoring the natural appearance of the eye, and minimizing further risk to the general health of both eyes. Enucleation is performed by ophthalmologists. They ensure the right time and decision and go over all risks and benefits prior to the procedure.

What is Marsupialization?

Marsupialization is a surgical procedure to treat Bartholin cysts. Bartholin cysts are lumps that consist of fluid, and they form on two small glands on either side of the labia. Bartholin cysts are formed due to a blockage in the opening on one of these glands, causing the fluid to lump up. With marsupialization, the Bartholin cyst is cut into a small slit, allowing the fluid to drain. It is then followed by a suture at the edges of the cysts to the surrounding tissue to form a small pouch. The pouch allows the fluid to drain freely from the Bartholin cyst.

Marsupialization is recommended if the Bartholin cyst is very painful and uncomfortable or if there is a history of returning a Bartholin gland cyst after treatment. The surgical procedure takes place in an operating room and lasts for about 10 to 15 minutes. It is carried out under local anesthesia or general anesthesia. It involves risks such as pain, infections, scarring, and hematoma. The patient is kept in the recovery room for a few hours before discharge. Antibiotics are prescribed to prevent infections.
